New Patient Intake Appointment 

Newly enrolled patients to Village Health are required to have a 30-minute appointment with one of our Health Professionals prior to seeing their enrolled Doctor. 

Cost: $40.00

What is included: 

  • Induction around Village Health Protocols, triaging and services 

  • Introduction to ManageMyHealth 

  • Lifestyle advice 

  • Review of your medical history 

  • Baseline observations - blood pressure check, height and weight 

  • Review current health concerns 

  • Prescriptions*

*depending on your medication list, you may be able to request a repeat prescription prior to seeing the GP. 

 

Please note your medical records will need to be available for this appointment. These can take up to 10 business days to come through from your previous practice. Please contact us to check if your medical records have arrived.

The New Patient Intake Appointment is not required for children under 18.
